WICKED WHIMS MOD
I recommend starting out with Wicked Whims Mod pregnancy system. This is an adults only mod but the game play features incredibly realistic. The mod is updated frequently with a lot of support if you run into any issues.
You can adjust the settings so your sim will have a menstrual cycle with chances of getting pregnant revolving around this. You can also purchase birth control is you wish not to get pregnant right away, pads for your sim’s period, encounter a miscarriage.
To become pregnant you can use the mod for realistic woohoo. If your sim seems to not be able to get pregnant there are fertility treatments they can take to help.
ULTRASOUND MOD
When your sims is pregnant in the 2nd or 3rd trimester you can use this mod to visit (rabbit hole) Gynecologist. When they return from the doctor they will have an ultrasound scan in their inventory. You can hang this the same as any picture in game.
PREGNANCY MOOD BUFFS MOD
Give your sims a wider range of emotions when they are pregnant with the mod. Instead of just “happy”, they will now feel sad, angry embarrassed, stressed, or uncomfortable.

PREGNANCY OVERHAUL MOD
If you don’t like the stomach size of your sims when they’re pregnant, walk style, or limited activities then this mod is for you. It will reduce the overall size of the pregnant stomach, just be aware that some interactions will look a little off do to the reduction in size. The pregnancy walk style is removed to just normal. And they can now do things such as swim, punch the punching bag, do yoga, dance, and much more.

LABOR PUDDLES MOD
Now your sim’s water can break right before it’s time to give birth. Your pets will also have labor puddles with this mod. You’ll get a puddle for each contraction your sim experiences.

FUNCTIONAL CRIBS
Once you have your baby you’ll be treated to a child that’s actually an object but at least you can now place them in more places including cribs, beds, etc. You can actually place them anywhere you want.
